TOETREDINGSVEREISTES: 

1. Om te kan deel in hierdie groepskema moet u ‘n lid van die TDVV wees en U jaarlikse fooi van R200.00 moet betaal en op datum bly.

2. Voltooi die onderstaande vorm vir die TDVV admin kantoor om U informasie na te gaan en status as lid van die TDVV te verifieer.

3. Korrespondensie sal via emails geskied so  maak seker dat U ‘n aktiewe email adres het vir hierdie doel

4. Die admin kantoor sal die vorms aan U stuur wat u moet voltooi en wat U dan na die Makelaar moet stuur wat alle aksies verder sal hanteer.

REQUIREMENTS: 

1.If you want to be part of this Medical Scheme, you need to be a member of the TSVA with a fully paid up yearly membership of R200.

2. Complete the form below and press the submit button. Our admin office will verify your membership and confirm your status as a TSVA member.

3. All correspondence is via emails therefore an active email address is a pre-requisite.

4. The application form will be sent to you by the admin office for completion and further instructions. You will forward the completed form to the Insurance Broker for processing.
